Symptom Management

Assistance with management of symptoms that are impacting on quality of life. For example symptoms experienced when living with life limiting illness, and in chronic disease.

Discussion of Goals of Care

We are able to provide assistance with family meetings to discuss goals of care. This can be done with your GP in attendance as a case consultation via telehealth or in person, whatever is most appropriate.

Education of health professionals

We provide educational sessions for health professionals in a number of areas or recommend an alternate provider to meet your needs.

Wound Care Consultancy

Both Jenelle and Fiona have undertaken further education in the area of wound management. They are able to offer advice and treatment options in regard to wound care. Ongoing dressings are continued with your primary health care provider.

Advance Care Planning (ACP)

Advance care planning is a routine part of a person’s health care. When a person’s values are discussed openly, their healthcare preferences can be respected at a time when they cannot voice their decisions. It’s an ongoing process that needs cooperation between individuals, their families, care workers, and health professionals as well as community organisations and healthcare organisations. (Advance Care Planning Australia). We are able to assist in the discussion or in helping prepare legal documents.

Medication Reviews

As Nurse Practitioners we are able to prescribe and de prescribe according to a formulary of medications within the scope of practice of the Nurse Practitioner – this is done in collaboration with your medical team.

Early palliative care referral 

Assessment and referral to Specialist Palliative Care if needed. Specialist palliative care is needed when your needs can not be met by your current health care providers.

Medical work up

If you are in need of a specialist medical assessment; Nurse Practitioners are able to order any required pathology or relevant assessments. This will enable the medical specialist to have access to results to guide care at your initial assessment with them.

Education of family members

We can assist family members in growing their understanding of an individuals care needs.

Referrals to other services

As a persons health changes so do their needs, as part of any assessment we undertake we will advise you of the most appropriate services available. We have a wide network of highly skilled health professionals that we are able to connect you with.

Mentoring Health Professionals

We offer mentoring to other health professionals particularly nurse practitioner candidates – including assistance with portfolio preparation for endorsement with APHRA.

This service is billed privately fee for service and costs are discussed according to time spent and needs of the individual.

Where are our services offered?

Our service is predominantly available to those living in the Eastern Region of Melbourne, Australia. If you live outside this area and need help we may be able to offer an alternate method of consultation, or if there are a number of people to see at one facility this may be possible.

We offer face to face consultation, at GP clinic, at your home, or other safe location or in residential aged care setting.

You are welcome to have another person with you to support you through the assessment – though we request no more than two additional persons accompany the person being assessed.

Consultation can also occur over the telephone or via telehealth.

How much does it cost?

In addition to the medicare rebate an affordable out of pocket fee applies to some services. Any applicable fees are discussed with you at the initial contact.

Consultations are provided mostly Monday to Friday during business hours. If you require a visit out of these hours or days these will attract additional charges.
